before finalizing your colors decision? If you are choosing a pre-approved color scheme
from this website, or from the VMR Color Scheme book at Qwal/Howell Paints, you
do not need to submit this form. Just buy your paint, or hire your contractor, and paint away!

If you are choosing colors or schemes that are not pre-approved by VMR, you must
submit this form, and additionally, you must also submit actual paint-color samples of the paint
you wish to use. After submitting this request, mail your paint samples and copy of your
